Services: Self-Pay Online / Order Your Test(s) Online

In an effort to make your lab testing needs more convenient for you, we are able to receive online prepayment for your lab test(s), then provide you with a prepaid Authorization Form that you can either bring to us or take to the Quest specimen collection site closest to you. Using this method of obtaining a lab test not only makes it incredibly convenient for you, but also allows you to receive the same self-pay savings you'd receive if you had done a walk-in appointment with us.

To order your test(s) online, click the button to the right called "Go to Online Order Form", and follow the directions on the form. Once you've completed your order, print the form you receive in your email and take it to the collection site of your choice; as proof of prepayment and for the lab's records.

Once we receive your test results, you will be notified. We have several network doctors that we can forward your test results to, or you can request one of your own.

We are continually adding tests to our list, so check back regularly. If there is a test you are interested in and can't find it on our list, please CALL or EMAIL our office.

We have customize and tailored all our services to accommodate everyone's needsWe have affordable prices for our self-pay patients and have the ability to price-match most tests.

Once we receive your test results, you will be notified. We have several network doctors that we can forward your test results to, or you can request one of your own.

We are continually adding tests to our list, so check back regularly. If there is a test you are interested in and can't find it on our list, please CALL or EMAIL our office.

 

As a self-pay patient, you have the option of a walk-in or scheduled lab collection visit. To schedule your lab collection visit, please call our office or click the "Schedule Request" button to the right. Please note that the appointment you create through our Online Scheduler does NOT set the appointment, but it does reserve your selected time slot. Once our office approves your requested appointment time, you will receive an email confirming your appointment time. 

We cannot guarantee that you can be served if you arrive at the time you requested on our scheduler before our office approves it, and you may be served as a "walk in" patient, instead.

Services: Mobile / Travel to Patients

Mobile services are by appointment only, and have the following fees added to any tests that you order. Note that you will only be charged a Mobile Service Fee only once per visit, regardless of the number of tests that you order. Mobile services are available to both self-pay and insured patients. Our Mobile Service Fee, plus all test costs, must be prepaid prior to our departure.

Mobile Service Fees

$50 Travel Fee + $0.57 per mile (up to our travel limit of 30 miles)

$35 Mobile Technician Fee
